New Patient Scheduling

First Visit

During your first visit to Bellevue Dental Health, we will introduce you to your dental team. We will collect and evaluate your medical and dental history. Feel free to download and print New Patient Forms if you would like to complete some of the necessary paperwork ahead of time to bring to your first visit. Your first appointment will consist of a comprehensive exam, necessary cleaning treatment, and any required radiographs for us to better understand your oral health needs.

Please assist us by providing the following information at the time of your first visit:

A list of your current medications. If you have dental insurance, please bring your dental insurance card or a copy of the front and back of the card. This will save time and allow us to help you process any claims. Current dental x-ray images, if applicable. If feasible, your completed New Patient Forms. IMPORTANT: A parent or guardian must accompany all patients under 18 at the exam.

X-Rays

If your dentist has taken x-rays, you may request that he or she forward them to our office. If there is not enough time, please pick them up and bring them to our office for your visit. If additional films are necessary, they can be taken at our office. We follow the guidelines set by the American Dental Association for partial and full-mouth diagnostic radiographs. It is recommended that cavity-detection x-rays are performed at least every 12 months and panoramic or full mouth x-rays are taken every three years.

Scheduling

Our office is open Monday: 9am – 5pm, Tuesday & Wed: 8am – 4pm and Thursday :7am – 2pm. We are also open for evening appointments one night a week as Dr. Libbi is sensitive to the busy lives and work schedules of her patients. This late evening rotates each week.

We will schedule your appointment as promptly as possible. Our office will make every effort to respect your schedule and to see you at your designated appointment time. If you have pain or an emergency situation, every attempt will be made to see you that day.
